United Home Life - New Final Expense App




New Final Expense App - Effective 10/1/11
One app - four products: Express Issue Whole Life, Deluxe, Premier AND GIWL!

Approved in all states in which UHL/UFFL accepts business EXCEPT Maine, Minnesota, Nevada & Virginia.*

 

What's different?:
  • 2-year look-back for all health, alcohol, drug and driving record questions for Express Issue Deluxe WL and Express Issue Premier WL.
  • Drugs prescribed for preventive or maintenance purposes to treat cancer, heart or circulatory conditions don't automatically disqualify applicants.
  • Previous declines/postpones from other carriers AND clients under 65 who are/have been disabled in the past 6 months can apply for day one coverage with Express Issue Deluxe WL.
  • Clients unable to qualify for Express Issue WL may qualify for GIWL1 without completing a separate app.
  • Applicants can authorize the Company to automatically issue an alternate plan of insurance (if they qualify) and adjust the face amount, if necessary.
  • Reformatted signature lines/authorizations make it easier to complete the app.
  • Note: Changes to health questions apply solely to Express Issue WL, Express Issue Deluxe WL and Express Issue Premier WL in states in which the new Final Expense application is approved (app form: 200-679 (UHL); 18-679 (UFFL)). These changes do not apply to any other UHL/UFFL product or application.

 

*UHL/UFFL not admitted in AK, NY, SD, or WY. UHL/UFFL does not accept business from NH or MT. Old final expense and GIWL apps will be accepted through 10/31/11. Old apps received after 10/31/11 will be returned to the agent. For states in which the new app is pending approval, the 10/31/11 cut-off does not apply. Product availability/application forms vary by state.
